Consents

The Consents page of the Member Consent service displays the consents that you have issued to your authorized representatives/applications to allow them to access your medical data and to act on your behalf for eligibility purposes.

On the Consents page, you can:

  • View the total count of the issued consents.
  • View and filter the number of consents with a certain status (Draft, Signed, Invalidated, Active, Revoked, Expired).
  • Search for the consents by the name of an authorized representative to whom these consents have been issued and by the effective and expiration dates of the consents.
  • View the list of all the consents present in the Member Consent service and their corresponding information, such as status, to whom these consents have been issued, effective and expiration dates, relationship with the person to whom this consent was issued.
  • Download the consent issued to a specific authorized representative.
  • View the consent as a PDF file.
  • Revoke a certain consent with the Active status.

To allow your authorized representatives to access your medical data from your health plan, issue an individual consent for this particular person.

To issue an individual consent:

  1. On the Consents page, click Issue Consent.
  2. In the Issue Consent dialog, in Consent Form, from the drop-down list with consent forms, select the consent form (Consent Form or General Consent) you require. You can select only one consent form from the list.
  3. In Authorized Representative, from the drop-down list, select the authorized representative to whom you want to issue the consent. You can select only one authorized representative from the list.
  4. In Consent Period, in Effective Date, enter the start date of the consent. If you have not specified any date, then the current date is populated by default for the first time when you save the consent.
  5. In Expiration Date, select the date of consent expiration (One year from Effective Date, Never expires, Custom Date). If you select Custom Date, then in Custom Expiration Date, select any date for expiration.
  6. Click Next. The consent is opened as a PDF file.
  7. Review the consent, select consent policy options, and then click Sign.
  8. In the Confirm Consent dialog, Member Name and Email are populated automatically.
  9. Select from the following two options: I hereby acknowledge that I have read and understood the consent details. or I am an authorized representative signing on behalf of the member..
  10. Click Sign, and then Close. The consent will appear in the list with consents and be assigned the Active status.
